We Educate you about Funded, Subsidized and Affordable Home and Community based Services, most of which are paid for through Government programs. We then assist you to request for person – centered services.
Personal Care Assistance (a family member can be paid to take care of you or your loved one if you do not want an outside care giver)
Transportation (to and from appointments and grocery shopping)
Nurse monitoring (a registered nurse monitors the care you receive to ensure you receive quality care and also monitor your health)
Home delivered meals (ready to eat meals)
Nutrition/Dietician (to advice on diet)
Durable medical equipment (wheelchair, walker, booster etc)
Disposable medical supplies (diapers, pullups, wipes, gloves, bed chux etc)
Medical day care ( a day program for health management and socializing, if needed)

Support planning (case manager to coordinate and monitor services)
